How reproducible:
constantly after mdadm was updated to 4.3-1.el9 in Centos 9 Stream
does not reproduce with mdadm 4.2-12.el9

We have the next configuration: KVM virtual machine with two sata disks, three md raid1 arrays for efi, boot and /
