OpenShift Console: "Create server adapter" has no effect

      1. ASSERT: make sure you have a valid account in your OpenShift Express Console
      2. ASSERT: make sure you have an application in your account
      3. EXEC: select your applicatoin and choose Create server adapter from the context menu
