Ensure proper behavior of cache loader miss and hit counts

It was brought up in the discussion here https://github.com/infinispan/infinispan/pull/3255#discussion-diff-25867061 that cache loader counts are not properly counted. We should decide if we want to count hits and misses from all accesses or not.
