Invalid Hostname showing in App Server list

When in the app server list, the server hostname is sometimes wrong or incomplete. this is due to it relying on the server ID, but it may not contain the hostname at all, or be in a different form.

With HAWKULAR-391 it's now possible to get the hostname from properties, that should be used.
