Allow custom policy failure formats

Currently we write the policy failure (and internal errors) for a request in a common apiman format (we support both JSON and XML). Some users have requested to be able to customize the output format. We should implement some sort of (optional) templating system to allow this customization. It would allow users to choose a policy failure template format per API.
