Add an index.html at the root of the manager micro-service to explain /apiman vs. /apimanui

Create a static index.html file that will live at the root of the apiman manager microservice. This index.html file will explain that the manager's REST API is located at /apiman and the UI is located at /apimanui. Bonus points for including the version of apiman in the file.
