Move the SimplePush and UnifiedPush examples to the real js-cookbook

currently the simplepush and unifiedpush examples, https://github.com/aerogear/aerogear-simplepush-quickstart and https://github.com/aerogear/aerogear-simplepush-unifiedpush-quickstart

are currently submodules in the aerogear-js cookbook.

we should move them into the actual repo and get rid of the submodules
