Ensure the certificate-authority-account create-account operation returns an appropriate message if the account already exists

If an account has already been created with LetsEncrypt and the create-account operation is called again, ensure that an appropriate message is returned so the user is aware that the create-account operation won't have an effect and that update-account should be used to make any further changes to the account.
