Ability to attach docs to process instance in the instance view.

Description of problem:

being able to add a document in the process instance view. This is critical for case manager where you can add documents on the ad-hoc during the process execution.
This is typically done by process owner to add additional documentations sometimes obtained through manual process like loan application additional documentation etc.
