Steps to Reproduce:

From Kie-Workbench Process Authoring of jbpm6.1

1) Create a simple process with a Start --> human task --> End
2) Encapsulate it in a simple swimlane
2) On BPMN-Diagram Properties add some Variables (Variables Definitions Property)
3) On Human Task Properties create some DataInputSet and DataOutputSet and assign (is mapped to type) them to Variables of (1) in Assignments Property
4) Generate all forms and then "Save and Deploy"
5) Execute the process from Process Definition Menu' and try to complete human task, It will not show up any form

then:
5a) Abort all Istances and go on Process Authoring again
5b) Remove just the swimlane
5c) Save, Deploy and execute again the process, now will be available the human task form
