Flag an exception in CachedConnectionValve if CachedConnectionManager is unavailable

If the CachedConnectionManager dependency in the Tomcat service is commented out and the CachedConnectionValve is active, there is a NPE.

It is better to throw an RuntimeException suggesting the user as to what needs to be configured (uncomment the CCM dependeny).
