CMT EJB should throw an exception if transaction marked rollback

Need the ability for an EJB with CMT transactions to detect that a transaction
is marked rollback (e.g. a Tx timeout) but the EJB does not itself throw an
exception.

This is so the exception can be signaled to the client that the transaction
did not suceed.
