CacheLoader/Writer.init called three times

PersistenceManagerImpl.createLoadersAndWriters

calls LevelDBStore.init three times unnecessarily.

Also note that LevelDBStore is both CacheWriter and CacheLoader and the logic of createLoadersAndWriters doesn't seem to take into account that these are not exclusive.
