Remove DB name from topic ids

Currently we add the DB name (PDB or CDB) to CDC topic names, resulting in four-part names, e.g. server1.mypdb.myschema.mytable.

That's different from other connectors, and also it seems to be a good idea to use a single connector for a single DB, in which case the logical server name would represent the captured DB.
