Asciidoc block titles are rendered the same as regular text

Specifically in the mysql.adoc reunify (and likely elsewhere) we have some block titles that are saved as:

.title

This is some text

But what happens when this gets rendered is more akin to:

title
This is some text

As can be seen, the block title isn't formatted to appear any differently from the text and this can lead to not being clear that sections exist.
