Prepare revised SMT docs (filter and content-based routing) for downstream

Per DBZ-2549, the filter and cbr SMTs were removed from the core Debezium package and reverted to TP. Updates were made in the upstream doc to reflect that change. To prepare the content for rendering downstream the following additional changes are required:

• Insert annotations to support modularization.
• Standardize style and language per RH CCS guidelines.
