When PostgreSQL schema refresh fails, allow error to include root cause

Whenever a SQLException is thrown inside PostgresChangeRecordEmitter#refreshTableFromDatabase(TableId), the caught exception does not maintain the root cause portion of the error thus its hard to diagnose the real reason this happened for the user.
