Add design description to SqlServerStreamingChangeEventSource

The class-level comment should describe the overall approach taken by that class:

• scanning of the CDC tables (how it chooses from which one to poll)
• how does it deal with schema changes
• etc.

Nothing fancy or too long is needed, but I'm looking for something a bit more high-level than the code itself.
