Steps to Reproduce:
Hide

Create a Plan A, lock it

Create a Plan B, lock it

Create a new service, configure all aspects of it

Select both Plan A and Plan B on the service's Plans tab

Publish the service

Navigate to the service's Overview tab

Reload the browser page (F5)

Navigate to the "Plans" tab

From now on, any navigation will show the dirty prompt

In particular, this seems to happen routinely on published services. Published services cannot be modified, so they cannot ever be dirty.
